Does Insurance Cover Rehab?

Yes.

Covered in nearly every case

In nearly every case, yes. Federal law is on your side here. The Mental Health Parity and Addiction Equity Act requires most health plans to cover substance use disorder treatment at the same level as other medical care,[1] and the Affordable Care Act lists addiction treatment as an essential health benefit that marketplace and Medicaid expansion plans must include.[2]

What changes from plan to plan is not whether treatment is covered, but how much of it: which levels of care, for how many days, and with what share of cost left to you. That is exactly what our verification process answers for your specific plan, in plain language, before you make any decision.

Tell Us What the Pattern Looks Like, We Will Map the Way Out

You do not need to know exactly what you took or have the right words for it. Tell our admissions team what the nights and the mornings after look like, and we will explain what assessment, detox, and treatment would mean for your situation, in one confidential call.

What Levels of Care Does Insurance Cover?

Most plans we work with cover the full continuum of care at Radix, subject to medical necessity review. That matters because recovery rarely happens at a single level of care. A plan that covers detox but not the residential treatment after it leaves you stranded at the most vulnerable moment. When we verify benefits, we map your coverage across every level at once:

Medical detox

Medical provider-supervised withdrawal management with 24/7 nursing.

Residential inpatient

30, 60, or 90-day live-in treatment at our Cedar Rapids facility.

Partial hospitalization (PHP)

Full-day clinical programming, 4 to 8 hours daily.

Intensive outpatient (IOP)

3 days per week of structured therapy around your schedule.

Continuing care

Long-term relapse prevention and low-intensity outpatient support.

What Will I Pay Out of Pocket?

Your out-of-pocket cost depends on your specific plan, not just your insurance company. Three numbers on your plan documents determine most of what you will pay:

Deductible

The amount you pay before your plan starts sharing costs. If you have already met part of your deductible this year through other medical care, your remaining responsibility shrinks.

Coinsurance or copay

Your share of costs after the deductible is met, usually a percentage of the allowed amount or a flat fee per service.

Out-of-pocket maximum

The most you can pay in a plan year. After you hit this number, your plan pays 100% of covered services. Many residents who complete a full continuum of care reach this cap, which means later levels of care often cost little or nothing.

When we verify your benefits, we walk you through each of these numbers for your exact plan, so there are no surprises.

How Does Insurance Verification Work?

1

Call or Submit Your Information

Call (319) 270-2890 or complete our confidential verification form. Have your insurance member ID card handy; the process takes only a few minutes.

2

We Verify Your Benefits

Our admissions team contacts your insurance directly, confirms your behavioral health benefits, and identifies your deductible, coinsurance, and any authorization requirements. You get a clear explanation of what your plan covers before you commit to anything.

3

Admit and Begin Treatment

Once benefits are confirmed and a clinical assessment is complete, most clients can admit the same day or the next day. We handle the paperwork so you can focus on getting well.

What If I Don't Have Insurance, or My Plan Won't Cover Enough?

You still have options. Our private pay program offers transparent self-pay pricing, payment plans, and private executive rooms for those who want them. If you qualify for Iowa Medicaid, we accept all three of the state’s managed care plans: Iowa Total Care, Wellpoint, and Molina. And if your plan covers treatment out-of-network, our team can pursue those benefits on your behalf.

Where Your Coverage Takes You

Every level of care lives under one roof in Cedar Rapids, with one clinical team and one billing relationship. When we run your free verification, we confirm benefits across the full continuum at once, so you know what each step would cost before you ever step down.

All six levels of care one campus in Cedar Rapids, one clinical team.

Most clinical → Independent

Medical detox

Medical provider-led withdrawal management with 24/7 nursing, typically over a 3 to 7 day window. As the most clinically intensive level, detox is verified alongside every other step when we check your benefits.

Residential inpatient

Living on site with structured programming and dual diagnosis care. Length of stay follows your clinical plan, and your coverage for this level is confirmed up front.

Partial hospitalization (PHP)

Four to eight hours of clinical programming daily with off-site living, a step down in intensity that we verify in the same benefits review.

Intensive outpatient (IOP)

Three days per week of therapy and medication management built around work and family, covered across the same continuum.

Standard outpatient

Weekly therapy and relapse prevention for as long as it helps, the most independent level of ongoing clinical support.

Continuing care and alumni

Long-term connection to the alumni community, recovery events, and a team that stays reachable after your formal treatment ends.

Your plan does not lock you into a single level of care. We verify your benefits across all six levels at once, so a single free call tells you what your full path of care would cost, not just the first step.

Common Questions About Rehab Coverage

Yes, in most cases. Federal parity law requires most health plans to cover substance use disorder treatment comparably to other medical care, and the ACA makes addiction treatment an essential health benefit. The details, which levels of care and what share of cost, vary by plan, which is why we verify your specific benefits free before you commit to anything.

Most plans cover residential inpatient treatment when it is medically necessary. Your insurer may require prior authorization and periodic reviews during your stay. Our admissions team handles authorizations directly with your plan so coverage is confirmed before you admit.

We work with Wellmark Blue Cross Blue Shield, Cigna, TriWest, Midlands Choice, Medical Associates Health Plans, Health Choices, and all three Iowa Medicaid managed care plans: Iowa Total Care, Wellpoint, and Molina. We also work with out-of-network PPO benefits and offer private pay options.

Yes. We accept Iowa Medicaid through all three of the state’s managed care organizations: Iowa Total Care, Wellpoint, and Molina Healthcare of Iowa, for qualified individuals ready for real change. Call us and we will confirm your plan’s coverage the same day.

Call (319) 270-2890 or submit our confidential verification form. Our admissions team contacts your insurer directly, confirms your behavioral health benefits, and explains your deductible and coinsurance in plain language. Verification is free and carries no obligation.

It depends on your deductible, coinsurance, and out-of-pocket maximum, not just your insurance company. Many clients with in-network plans pay only their remaining deductible and coinsurance, and those who reach their out-of-pocket maximum pay nothing further for covered care that year. We give you exact numbers for your plan during verification.

Yes. Our private pay program includes transparent pricing, payment plans, and private executive rooms. If you may qualify for Iowa Medicaid, we can point you toward enrollment resources and accept all three Iowa Medicaid plans once you are covered.

Most clients can admit the same day or the next day after benefits are verified and a clinical assessment is complete. Our admissions team works around the clock, so an evening or weekend call does not slow anything down.
